Remove init of proto, header field, expert info and subtree variables.
This will reduces the binary size by approximate 1266320 bytes due to
using .bss to zero-initialize the fields.
The conversion is done using the tools/convert-proto-init.py script.

Only dissectors are using this function and there is no use case,
as far as I know, that requires its use. Any limitation of length
is imposed transparently by the UI backend.
This function is problematic because it is not Unicode aware and
will truncate a string on an arbitrary byte boundary for multibyte
strings.
Replace its use with a normal strbuf without a length limite and
remove the function because it is not useful and the ITEM_LABEL_LENGTH
parameter does not belong in wmem anyway.

Do not require a useless ENC_NA parameter for string encodings.
FT_STRING and FT_STRINGZ types don't have any ndianness.

Automated find/replace of wmem_packet_scope() with pinfo->pool in all
files where it didn't cause a build failure.
I also tweaked a few of the docs which got caught up.

They already know who they are when they register themselves. Saving the
handle then to avoid finding it later.
Not sure if this will increase unnecessary register_dissector functions
(instead of using create_dissector_handle in proto_reg_handoff function)
when other dissectors copy/paste, but it should make startup time
a few microseconds better.

Similar to the "tcp.port" changes in I99604f95d426ad345f4b494598d94178b886eb67,
convert dissectors that use "udp.port".
More cleanup done on dissectors that use both TCP and UDP dissector
tables, so that less preference callbacks exist.

The preferences are still supported for backwards compatibility, but the heuristic_protos file has final say on the "preference" to enable/disable a heuristic dissector.
Also add parameter to heur_dissector_add() for the "default" enable/disable of a heuristic dissector. With this parameter, a few more (presumably weak) heuristic dissectors have been "registered" but of course default to being disabled.

Specifically:
- Set packet.h to be the first wireshark #include after
config.h and "system" #includes.
packet.h added as an #include in some cases when missing.
- Remove some #includes included (directly/indirectly) in
packet.h. E.g., glib.h.
(Done only for those files including packet.h).
- As needed, move "system" #includes to be after config.h and
before wireshark #includes.
- Rework various #include file specifications for consistency.
- Misc.

Hopefully that name makes it clear what the routiner's purpose is, and
will encourage people to use it rather than using dissector_add_uint()
with a bogus integer value.

hf_ field name is the first part of the formatted string. This was done with a perl script on the dissectors directory (packet-*.c), followed by manual inspection of the output. The manual inspection yielded a few cases that really should have been proto_tree_add_uint or proto_tree_add_item, so I updated them accordingly.
The script didn't catch as many as I would have liked, but it's a start.
The most common (ab)use of proto_tree_add_uint_format was for appending strings to CRC/checksum values to note good or bad CRC/checksum.

This patch protects against malformed Unique ID lengths (>= 252 bytes) and
defer all tvb_xxx related calls to when we actually need them. This allows us
to dissects as much as possible before bailing out (in case of a malformed
length).
From me:
Add a link to the protocol specification. Remove packet-miop.h along
with some struct definitions. (Don't tvb_memcpy over structs!) Remove
some more tvbuffs. Don't call proto_item_add_subtree if we're not going
to use the trees. Call proto_tree_add_item instead of
proto_tree_add_text Remove a bunch of unused ett_ variables. Add an
expert item for the protocol version. Register the field array with the
correct length.
