Update .gitignore and make the manuals build without a toplevel
configure.ac file from autotools:
$ cd doc/manuals
$ make

Moved to doc/manuals/, with full commit history, in preceding merge commit.
Now incorporate in the build system.
Build with:
$ autoreconf -fi
$ ./configure --enable-manuals
$ make
Shared files from osmo-gsm-manuals.git are found automatically if
- the repository is checked out in ../osmo-gsm-manuals; or
- if it osmo-gsm-manuals was installed with "make install"; or
- OSMO_GSM_MANUALS_DIR is set.

Begin with manual for the layer23 ("mobile") application but focus
on the scripting code first.

Link to Lua docs, link to our manual. Demo logging, timer, MS
on/off, sms sending and sms receiving. For the MM state I will
need to export constants/names to make this usable. Currently
it is a bit of a hack (first time to MM_IDLE) to send a SMS. We
would know when we attached though.

It is better to have a shared example config files directory,
like other Osmocom projects do.
