Misc dissector cleanup:
- Make some fcns & vars static
- hf[] blurbs: "" and repeated text --> NULL
- Move proto_register & proto_reg_handoff to end of source
- packet-catapult-dct2000: simplify proto_reg_handoff
- Use consistent indentation
