Fix proto_tree_add_item() encoding parameter:
- Use ENC_BIG_ENDIAN instead of a (non-sensical) variable value;
- Use ENC_NA for hf field type FT_BYTES;
Also:
- Remove unneeded #includes;
- Use consistent indentation;
- val_to_str_...() ==> val_to_str_..._const()
